We offer flexible, person-centred outreach support for individuals with Learning Disabilities and/or Autism Spectrum Disorder. Our outreach services are designed to promote your independence at home and help you actively participate in your community.

We work closely with you and your support network to ensure that the services provided are personalised and aligned with your goals. Outreach can sit alongside family care, complement supported living, or stand alone — whatever level of support fits your life right now.

Outreach support is flexible by design. Together we agree how many hours suit you, which goals to focus on first, and how we'll know it's working — then we review and adjust as your confidence grows.
